OVERVIEW

We are currently seeking a skilled and compassionate Telehealth Nurse Practitioner, licensed in multiple U.S. states, to join our growing virtual care team. This role plays a vital part in providing services such as medication therapy management, chronic care support, and care coordination in close collaboration with partnering health centers. We immediately licensed Nurse Practitioners in Texas, Illinois, Connecticut, Indiana, New Hampshire, California, and Georgia.

MUST HAVE AN ACTIVE LICENSE IN ANY OF THESE STATES :

Texas, Illinois, Connecticut, Indiana, New Hampshire, California, and Georgia.

RESPONSIBILITIES

  • Conduct Virtual Assessments and Consultations
  • Perform video or phone-based evaluations, reviewing patients' medical histories, medication regimens, and current health status.
  • Provide Patient Counseling and Education
  • Deliver guidance on preventive care, chronic disease management, lifestyle modifications, and medication adherence.
  • Coordinate Care and Make Referrals
  • Work with health center care teams, specialists, and community-based providers to ensure seamless coordination of care, including necessary referrals and follow-ups.
  • Maintain Documentation
  • Accurately document patient encounters and care plans in the electronic medical record system in a timely and compliant manner.

Requirements

THE IDEAL CANDIDATE

  • Active Nurse Practitioner license in multiple U.S. states - APRN
  • Proficiency with EMR systems and virtual care platforms
  • Background in primary care, chronic care, and medication reviews
  • Strong communication skills and ability to work both independently and within a collaborative care team
  • Flexibility to provide care outside standard business hours occasionally
