Pharmacist (Hawaii – Remote)
Work Arrangement: Fully Remote
Target Location: Hawaii (HI)
Licensure Requirement: Active pharmacist license in Hawaii required
Role Overview
A forward-thinking telehealth organization is seeking a Pharmacist to support patients across Hawaii through remote clinical services.
This position is centered on virtual care delivery, with a strong emphasis on medication optimization, patient education, and collaboration with interdisciplinary care teams to improve health outcomes.
This role is ideal for pharmacists who are adaptable, patient-focused, and confident in leveraging digital tools to deliver high-quality care.
Key Responsibilities
- Provide comprehensive Medication Therapy Management (MTM) services
- Conduct medication reconciliation and assess therapy effectiveness
- Deliver patient-focused education on medications and chronic conditions
- Support chronic disease management (e.g., diabetes, hypertension, COPD, CHF)
- Utilizemotivational interviewing techniques to drive adherence and engagement
- Collaborate with providers to optimize medication regimens and care plans
- Identify and resolve medication-related issues and risks
- Document patient interactions accurately and in compliance with program standards
- Support care transitions and continuity of care initiatives
Required Qualifications
- Doctor of Pharmacy (PharmD)
- Active pharmacist license in Hawaii (HI)
- Willingness to obtain additional state licensure if needed
Preferred Qualifications
- Board certification (e.g., BCPS, BCGP) strongly preferred
- Experience in:
- Medication Therapy Management (MTM)
- Ambulatory care or outpatient settings
- Chronic disease management
- Patient education and engagement
- Familiarity with care programs such as CCM, TCM, and RPM is a plus
